Providence St. Joseph Health Clinical Pharmacy Specialist *Remote* in Vancouver, Washington

Providence Health Plan is calling a Clinical Pharmacy Specialist who will: Be responsible for the effective provision of clinical pharmacy services to members of Providence Health Plan (PHP) Collaborate with medical directors, nurse case managers, and network physicians and other providers to provide coordinated, high quality and evidenced-based, cost-effective care to PHP members Be responsible predominantly for the following areas: Direct patient care through Medication Therapy Management (MTM) program Population-based disease management Specialty pharmaceutical acquisition and care coordination Drug utilization review and program development Clinical policy development and application, including Pharmacy and Therapeutics (P&T) Committee participation Be skilled at reviewing pharmacy and medical claims Interact with, assess and provide education to members and caregivers Develop medication-related problem lists Implement medication action plans including interventions with health care providers Educate and consult with physicians, pharmacists, nurses, and other members of the health care team, both internally and externally Be responsible for drug utilization work and medical policy activities including developing and applying drug medical policies to influence safe, quality, and cost-effective use of prescription pharmaceuticals Please note the following important details regarding this opening: We are hiring for multiple openings of a Clinical Pharmacy Specialist We welcome 100% remote work for residents who reside in one of the following States: Washington Oregon Required qualifications for this position include: Pharm D: Graduate from an accredited College or School of Pharmacy -OR- Bachelor's or Master's Degree in Pharmacy and 5+ years of managed care experience with with experience documenting responsibility for evidence and clinical case review and direct consultation of medication information with physicians and/or patients PGY-1 Residency training with 1 year of experience within relevant practice setting -OR- a PGY-1 Managed Care Residency with Providence Health Plan -OR- 3+ years' experience as a clinical pharmacist in managed care Oregon Pharmacist License National Certification from Board of Pharmacy Specialties within 2 years of hire date Experience in writing, including research protocols, medical papers for publication, and educational pieces for patients and healthcare providers Experience reviewing medication evidence and clinical policy development Experience working in programs and services that support population health management Preferred qualifications for this position include: Completion of specialty/PGY-2 Residency Experience and expertise working with a pharmacy claims processing platform Medicare Part D Stars experience Why Join Providence?
