    Diabetes Educator Details

    Essential Job Duties:

  • Performs comprehensive learning needs assessment with regard to physical, psychological, and environmental factors that may affect the plan of care.

  • Plan of care which includes goals and prioritized nursing/medical approaches derived from data collected.

  • Utilizes additional resources in goal setting and meeting learning needs.

  • Implements strategies to provide for patient's participation in goal setting, health maintenance and maximization of health capabilities.

  • Notifies physician of significant changes in patient's condition and potential or suspected complications.

  • Evaluates the patient for diabetes knowledge and status of diabetes management.

    Qualifications & Licenses:

  • Currently licensed by the Indiana State Board of Nurses' Registration and Nursing Education as a registered nurse with one year in diabetes education preferred.

  • Certification by the American Association of Diabetes Educators (CDE) required or willing to obtain within 2 years of hire. Possible extension may be granted of up to 6 months to accommodate testing availability depending on hire
